Capstone Data Review Project Proposal Outline Template
1. Cover Page with specific, descriptive title.
2. Table of Contents: Headings and subheadings with right-justified page numbers.
3. Executive Summary: One page, double-spaced.
4. Introduction: Four sentence topic description. (Statement of problem, factors to be evaluated, units of measurement for each factor, and value proposition or relevance statement.)
5. Statement of the Problem: (Please state the issue in the form of a problem).
6. Background: (Literature: six authoritative sources, summarized in short statements with citations. Wikipedia and commercial Web sites are not authoritative sources).
a. The issue from a national or industry level.
b. Current, relevant facts, trends, and emerging issues.
c. The issue relative to your subsector of the health care industry or continuum of care.
d. Data and statistics that substantiate the need to examine the issue (or, what are the consequences and risks of not addressing the issue?).
7. Client Needs Assessment.
a. Restate any pertinent information.
b. Make a precise statement about what needs to be done to investigate the problem (for example, data analysis) and how it will add value to the organization.

8. Expected Outcomes and Precise Performance Measurement.
a. What data will be collected and analyzed?
b. How will it be measured? Be precise in explaining how the outcome will be measured.
c. Provide a visual display of the proposed infrastructure for the summarized or grouped data. (For example: Pie chart, spreadsheet, or process map.)
9. Leadership Component.
a. Your precise roles and responsibilities in leading the effort. (Note: Human subject research is outside the scope of the Health Administration Capstone course.)
b. Select one ACHE competency from the list below. Then, indicate how it relates to your project.
1. Evaluate problems in health care organizations, and apply tools to improve quality and outcomes.
2. Analyze foundational elements of the US health care system.
3. Construct evidence-based health care management recommendations in compliance with personal and professional values and legal, regulatory, and ethical considerations.
4. Analyze records and reports based on established benchmarks and organizational goals and performance.
5. Create comprehensive and useable data-driven action plans, based on industry benchmarks.
6. Communicate effectively with diverse audiences, in an appropriate form and style, consistent with applicable organizational, professional, and scholarly standards.
10. Timeline: Include a week-by-week summary of major tasks to be done to complete your data review project.
