Monitor health status to identify community health problems.
The Cumberland County Department of Public Health along and Cape Fear Valley Health Systems conducts a Community Health Assessment (CHA) every three years to describe the health status and needs of the community. Data is collected through surveys and statistical data. (Cumberland County Department of Public Health, 2012). These assessments are essential in order to identify real community health problems.
